Our team works alongside individuals, families, case managers, and counties to plan and deliver the right blend of services — at home, in the community, and on the path back to community living. From our office on Washington Avenue N in downtown Minneapolis, we serve clients throughout the Twin Cities metro and across greater Minnesota.
