What 245D services include
Minnesota's 245D license sets the standards for home and community-based services that help people with disabilities and seniors live as independently as possible. Premier Service is authorized to deliver these supports as part of a person-centered plan.
The 245D services we provide
Our 245D licensure covers a comprehensive set of home and community-based supports β delivered in a person's home or in the community according to their plan.
- Individualized Home Supports (IHS) β daily, in-home support with and without family training, built around each person's plan.
- Homemaker Services β cleaning, home management, and personal cares that keep a home running smoothly.
- Night Supervision β overnight support in the person's home.
- 24-Hour Emergency Assistance β on-call support when something unexpected comes up.

- Transportation β to appointments, work, and community activities.
Funded through Minnesota waivers
Most 245D services are supported through Medical Assistance waiver programs β CADI, BI, CAC, DD, and EW β and coordinated with your county case manager.
Who is eligible
Eligibility typically runs through Minnesota waiver programs and county case management. The CADI and BI waivers help people who need a level of care that would otherwise be provided in an institutional setting. What working with us looks like
We start with a conversation, learn about goals and needs, then build a plan in partnership with the case manager. Our team handles documentation and coordination so families can focus on what matters most.

Do I need a waiver to receive services?
Most 245D services are funded through Minnesota Medical Assistance waivers. We can help you understand whether a waiver is the right path and connect you with your county case manager.
Can services be provided in my own home?
Yes. Most of our 245D services are delivered in people's own homes and in the community where they live, work, and connect.
How are staff selected and trained?
Our staff complete the required Minnesota DHS training and ongoing development in culturally respectful care, person-centered practices, and safety.
