Minnesota Statute 245D sets the legal framework for home and community-based services for people with disabilities and seniors. It establishes staffing requirements, training standards, documentation practices, and rights protections.
Premier Service holds an active 245D license issued by the Minnesota Department of Human Services. This means every service we deliver meets or exceeds the state's standards for safety, quality, and person-centered care.
Every service is delivered in the person's home or community — never in a facility.

Most 245D services are funded through Minnesota Medical Assistance waiver programs, coordinated with your county case manager. We accept all of the following.
Community Access for Disability Inclusion
For adults with physical disabilities who need a nursing-facility level of care.
Brain Injury
For individuals with an acquired brain injury who need a nursing-facility level of care.
Community Alternative Care
For individuals who need a hospital level of care.
Developmental Disabilities
For individuals with developmental disabilities who need an ICF/DD level of care.
Elderly Waiver
For seniors who need a nursing-facility level of care and choose to live in the community.
